To request an appointment with SHARE, students can contact SHARE directly by phone 609-258-3310 or complete an appointment request form through the MyUHS portal.

To access the appointment form:

1. Login to the MyUHS portal
2. Select "Forms" on the top menu
3. Scroll down the page to the SHARE section and select SHARE Appointment Request, as illustrated in the image below. 
4. Complete the online form.  All requests will be responded to within 24 hours, when submitted on a business day. All other requests will be responded to on the next business day.
